  • Insurance card
  • Driver’s license or Photo ID
  • Authorization payment and co-pay (for your convenience we accept Visa & MasterCard)
  • List of medications, over-the-counter medications, and vitamins including eye-drops

Missed Appointments

Be sure to arrive 15 minutes early for your appointment. If you are more than 15 minutes late for your appointment you may need to be rescheduled.
